Protocol & Liability Release:

I acknowledge that iAspire LLC is taking precautions to reduce the spread of the Covid-19 virus: Therapist protocol: -Is wearing a mask and/or gloves during services especially when requested by the client -Is cleaning all surfaces potentially touched within the office and bathroom with disinfectant or alcohol between clients. -Is opening windows to air out the treatment room between sessions, and staggering appointments to reduce client-to-client contact. -Each session is started with freshly washed/disinfected hands. Client Protocol: -I acknowledge it is left to my judgement and comfort level as to whether I wear a mask during a session. -I acknowledge that it is requested of me to thoroughly wash my hands prior to entering the therapy room. -I acknowledge that iAspire LLC shall no be held responsible or liable if I contract the Cover-19 virus, because I came in willingly to the office, knowing the precautions taken, but also the potential risk.
